We accept referrals from professionals or you may refer yourself.

You can contact our admission team to discuss accessing residential rehabilitation treatment with us. You can download a referral form from the link below or you can ask the team to send you one. Once completed you can email, post or fax the form to us. Once we have received your completed referral form, we’ll be able to arrange an assessment appointment (via telephone or a visit to the service).

All your information will be treated in a secure and confidential way in accordance with the General Data Protection Regulations 2018.
